After having done Insane in the Membrane five times, I get a lot of questions about this Feat of Strength. Here I have compiled the questions I most frequently get from players like you, along with the answers.
Q. What is so special about Insane in the Membrane?
A. Insane in the Membrane consists of achieving several difficult reputation grinds, none of which have associated rewards such as vendor items or separate achievements. Thus, players will never achieve these reputations in normal gameplay. The title derives its meaning from the extreme difficulty and cost of gaining the reputations for absolutely no reward. It is therefore classed as a Feat of Strength and grants a title, The Insane.
Insane in the Membrane is a reference to the 1993 Cypress Hill song Insane in the Brain. By the time you’re done, you’ll understand.
Q. How can I track my progress?
A. It is not possible to track your progress in-game. The in-game achievement tracker used to be able to do this, but it was broken in Patch 5.0, and removed entirely in Patch 6.0.
The data is still available in the armory, though, and we make it available on site here. You can track your progress by logging into this site with your Battle.net account and viewing your progress in the sidebar. The guides will also show you which relevant quests you have completed.
Q. Is Insane in the Membrane character-specific or account-wide?
A. Beginning with Patch 5.0.4, Mists of Pandaria, World of Warcraft now includes achievements which can apply to all characters on a Battle.net account. You must complete all of the reputations for Insane in the Membrane on a single character, but once you have done so, the Feat of Strength is visible, and you can use the title, on all of your characters.
Q. Do I need to become Exalted with Bloodsail Buccaneers?
A. No. To obtain Insane in the Membrane, only Honored is required. Some players may choose to obtain Revered or Exalted for their own personal amusement.
Q. Do I need to keep Bloodsail Buccaneers reputation at the same time as Steamwheedle Cartel reputation?
A. Once you reach Honored with Bloodsail Buccaneers, you can safely lose the reputation again. The only restriction is that you must have earned the reputation after April 14, 2014, or have obtained the Avast Ye, Admiral! Feat of Strength prior to that date.
Some Blizzard GMs have given incorrect answers to this question.
Q. I completed Bloodsail Honored during Mists of Pandaria, but I am not sure if I received credit for it. What do I need to do?
A. Due to a bug introduced in Patch 5.0.4, achievement tracking for Insane in the Membrane was broken during most of the expansion, and finally mostly fixed before patch 5.4.8. Upon fixing it, Blizzard retroactively granted credit for Bloodsail Buccaneers to anyone who completed the Avast Ye, Admiral! feat of strength, but players might not see this reflected in the in-game tracker. If you earned the Avast Ye, Admiral! feat of strength prior to April 14, 2014, or if you obtained Honored on or after that date, you have received credit for it. Otherwise you will need to obtain the reputation again.
Q. Can I use the cloth turn-ins to restore my Steamwheedle Cartel goblin reputation from Hated to Neutral?
A. Yes. While you will lose reputation with Bloodsail Buccaneers by turning in cloth to repair your goblin reputation, your progress toward Insane in the Membrane will be unaffected, so long as you previously reached Honored with Bloodsail Buccaneers. The only restriction is that you must have earned the reputation after April 14, 2014, or have obtained the Avast Ye, Admiral! Feat of Strength prior to that date.
You may choose to maintain your Bloodsail Buccaneers reputation at Honored if you wish, or you may choose to obtain and keep Revered or Exalted. Neither Revered nor Exalted are required for Insane in the Membrane, though reaching them still qualifies for Insane in the Membrane.

Q. I was killing pirates and when I got to Revered they stopped giving reputation. What’s going on?
A. Pirates which grant reputation with the goblin towns when killed only do so through Revered, though they grant spillover reputation with the other three towns all the way through Exalted. To finish the grind to Exalted you must kill pirates for a different town and collect the spillover reputation.
Q. Which monsters can I kill for Booty Bay reputation without losing Bloodsail Buccaneers reputation?
A. Venture Co. goblins in Northern Stranglethorn and Grizzly Hills grant direct reputation with Booty Bay without losing reputation with Bloodsail Buccaneers. You can also gain spillover reputation from grinding pirates for other goblin towns without losing Bloodsail Buccaneers reputation.
Q. I got my goblin reputations to Exalted during Mists of Pandaria, and lost them again. I am not sure if I received credit for them. What do I need to do?
A. Due to a bug introduced in Patch 5.0.4, achievement tracking for Insane in the Membrane was broken during most of the expansion, and finally mostly fixed before patch 5.4.8, on April 14, 2014. While Blizzard was able to retroactively grant credit for some players for Bloodsail Buccaneers reputation, no retroactive credit was given for any of the other reputations. If you obtained Exalted for any of the reputations on or after April 14, 2014, then you have received credit for it. Otherwise you will need to obtain the reputation again.
Q. At what level can I obtain Heavy Junkboxes?
A. Heavy Junkboxes are pickpocketed from level 50-60 humanoids and ghouls in Azeroth and Hellfire Peninsula, Outland. They can also be obtained from certain lower level mobs in Western Plaguelands and Eastern Plaguelands whose levels were lowered in Cataclysm but whose pickpocketing loot tables were not changed. These are sufficiently few and far between as to not be worth hunting down.
Completing the repeatable quest Junkboxes Needed requires level 50.
Q. Where is the Darkmoon Faire?
A. Portals in the Darkmoon Faire staging locations in Elwynn Forest and Mulgore will teleport you to Darkmoon Island. Darkmoon Faire Mystic Mages in Stormwind, Darnassus, Orgrimmar and Dalaran will teleport you to the portal locations, but will charge you depending on your character’s level, up to 50 silver at level 90.
Q. How did the Darkmoon Faire reputation grind change in Patch 4.3?
A. Darkmoon Faire Island offers several quests that can be repeated once per month. Completing all of the available daily and monthly quests every day the Faire is open will award 20,250 reputation with Darkmoon Faire, not counting reputation bonuses. Q. I want to obtain the Loremaster achievement. How should I combine it with Insane in the Membrane?
A. You should complete part of Cape of Stranglethorn Quests, excluding the quest line leading up to the Brashtide Invasion, and Arathi Highlands Quests before beginning any part of the Bloodsail Buccaneers or Steamwheedle Cartel reputation grinds, because these zones contain many quests which will reduce your Bloodsail Buccaneers reputation.
